A customized support network of daily services allows each resident to retain as much independence and autonomy as possible. Our highly trained, licensed or certified clinical staff are able to coordinate care, follow physicians’ orders, monitor health status and manage medications. Among the care services available are the following.

24-Hour Nursing

On a rotation basis, services from the licensed nurses on staff are available onsite 24-hours each day. This allows CSL communities to accept and retain residents with greater health-related needs as well as comprehensively manage their care. It also assures that a skilled professional is quickly available should emergencies arise.

Onsite Ambulance Service

Various ambulance companies post their vehicles, paramedics and emergency medical technicians at the communities. These professionals specialize in providing routine care and transport to seniors (including those with dementia), but are also on hand to offer emergency assistance, if needed.

Resident Liaison

At most Carlton communities, a full-time staff member is the “point person” between the caregiving staff and the families. Liaisons coordinate a thorough evaluation and assessment each resident receives upon move-in,. This information is utilized to develop individualized care plans. Ongoing “Family Connection” conferences help ensure that significant changes in resident health status are quickly noted so services can be adjusted accordingly.

Clinical Services Consultants

The communities’ wellness teams are supported and educated by CSL Home Office staff consisting of four full-time, dedicated consultants with expertise in clinical services management, policy development, dementia care and medications management. Communities can call upon these professionals when questions arise or when they would like added information, references and resources. Additionally, the consultants routinely monitors clinical operations for quality assurance.

Award-Winning Care Programs

The Assisted Living Federation of America (ALFA), the national trade association which represents assisted living providers, periodically bestows “Best Of” awards for programs of merit. So far, Carlton Senior Living has been honored with three awards, including:

Advanced Care Training (ACT), which provides caregiving staff with additional training on conditions and diseases common among the elderly. Increased understanding better enables them to assist with care related to strokes, diabetes, multiple sclerosis, arthritis complications, Parkinson’s disease and other disorders.

Diabetes Management Program, which is designed to assist residents to live better with their diabetes on a day-to-day basis. It includes services from licensed vocational nurses, medication management, diet and exercise plans and more.

Fall Awareness and Reduction Program, which includes fall risk assessment as well as balance fitness training, healthy walking classes and educational seminars to promote enhanced resident mindfulness.

LiFE Memory Care

Within five CSL communities is a smaller, quaint environment designed to provide superior care for seniors with dementia-related challenges. Residents are able to enjoy LiFE – Letting Individuality Freely ExistTM – a person-centered approach to care which values all people as unique, recognizes individual viewpoints and honors the connection among us all. Entrances and exits utilize delayed egress and monitoring technology. The program utilizes dementia care best practices as well as state-of-the-art sensory integration and therapeutic treatment tools. Chateau Poet’s Corner in Pleasant Hill is Carlton’s only community dedicated to memory care. Other communities offering it are Chateau III Pleasant Hill and the Carlton Plazas in San Leandro, San Jose and Sacramento.

Physical Therapy

Many communities have partnered with and offer onside physical therapy. Specially designed exercises, treatment and equipment help residents and other local seniors improve or regain physical abilities and mobility after a stroke, surgery or injury.

Hospice

All CSL communities allow onsite hospice care and services. The focus is on comfort and quality of life for the resident and his or her family during the final phase of a terminal illness.
