Carlton Plaza and Carlton Crown Plaza Sacramento, communities offering assisted living, are committed to the well-being of each resident. The State of California licenses assisted living communities, but allows each provider to determine the range of health-related care they will make available.
From its inception, Carlton Senior Living decided to offer the broadest spectrum of care allowable. This makes CSL better able to meet residents’ chronic and long-term care needs. Below is a description of the added services at Carlton’s Sacramento communities that allow them to go the extra mile in delivering exceptional care.
Medical Director
While under no obligation from the state licensing agency to have a medical director in an assisted living community, the Plazas have chosen to establish that relationship with Andrew J. Tak, M.D. His advice to the wellness team allows for cohesive oversight of each resident’s well-being. Board certified in internal medicine, Dr. Tak is a long-time practitioner in the Sacramento region. He prides himself on an approach to medicine that allows him “to meet patients varied needs and to care for their mind, whole body and spirit.” He also embraces the concept of “concierge medicine,” which allows his patients to receive more of his time and attention for an already negotiated fee that frequently lowers their annual spending on doctor’s visits.
Wellness Team
Staff nurses, both RNs and LVNs, are able to coordinate care, monitor resident’s health status, and follow physicians’ orders for injections, and manage medication. They also train personal caregiving staff and, at Carlton Crown Plaza, are available onsite 24-hours each day. This allows the Plazas to accept and
retain residents with greater health-related needs as well as comprehensively manage their care. It also assures that a skilled professional is quickly available should emergencies arise.
Both communities also manage residents’ medication using specially trained Medication Technicians and cutting-edge electronic medication administration records (eMARS) technology. This helps minimize medication errors, increase staff efficiency, provide comprehensive documentation and improve quality of care.
Resident Liaison
Our Resident Liaison acts as the “point person” for family members, so families can receive up-to-date information about their loved one and solve concerns that may arise. Family involvement in the life of the community is highly encouraged.
Clinical Services Consultants
The communities’ wellness teams are supported and educated by CSL Home Office staff—four full-time, dedicated consultants with expertise in clinical services management, policy development, dementia care and medications management. Communities can call upon these professionals when questions arise or when they would like added information, references and resources. Additionally, the consultants routinely monitor clinical operations for quality assurance.
Award-Winning Care Programs
The Assisted Living Federation of America (ALFA), the national trade association which represents assisted living providers, periodically bestows “Best Of” awards for programs of merit. So far, Carlton Senior Living has been honored with three awards, including:
- Advanced Caregiver Training (ACT), which provides the entire personal caregiving staff with additional training on conditions and diseases common among the elderly. Increased understanding better enables them to assist with care related to strokes, diabetes, multiple sclerosis, arthritis complications, Parkinson’s disease, and other disorders.
- Diabetes Management Program, which is designed to assist residents to live better with their diabetes on a day-to-day basis. It includes services from licensed vocational nurses as well as medication management.

Physical Therapy
David T. Clawson, M.P.T., maintains an office in Carlton Plaza where he uses specially designed exercises, treatment and equipment to help residents and other local seniors improve or regain physical abilities and mobility after a stroke, surgery or injury. He has practiced in the Sacramento region for over 20 years, including seven as director of rehabilitation in skilled nursing care.
Onsite Ambulance Service
AlphaOne Ambulance Service posts paramedics and emergency medical technicians at the communities. These professionals specialize in providing routine care and transport to seniors (including those with dementia), but are also on hand to offer emergency assistance, if needed.
Hospice
Both Carlton Plaza and Crown Plaza have received approval from the state licensing agency to allow hospice care and services. This support is made available to residents in the final phase of a terminal illness. The focus is on comfort and quality of life for the resident and his or her family.
